Applicant shall submit request in writing on or before
February 25th.

Applicant shall complete permit form, listing stops:
A.
List make and model of equipment
B.
List what will be moved
C.
Plate numbers of equipment to be used
D.
Proof of insurance
E.
Permit fee of $120.00 for multiple moves for milk haul route
under agreement with the Ingham County Road Commission. Single moves may be
obtained at a cost of $42.00 per move.

Approved permit shall be carried in vehicle at all times.

The Ingham County Road Commission shall determine routes to
be used during weight restrictions.

If the applicant’s vehicle is found off approved routes,
and/or overweight during restrictions, then the permit is null and void and the
applicant shall reduce his loads for the remainder of weight restrictions.

If applicant’s vehicle has air axles, they shall be down
while carrying a load.
